Getting started is simple. Either contact us and ask for a New Patient Pack to be mailed to you, or download and print it from our website Patient Resources Section. This is free and completely confidential. If we feel that there is need for additional preparation on your part before you begin the program, we will inform you of our recommendations. Once given the “green light”, we will facilitate the necessary steps to a start date.
Alternatively, we encourage you to schedule for a complementary tour with us. You may bring a completed questionnaire with you or mail it in advance if you wish to discuss proceeding to The Gilbert Clinic Health Restoration Program. During your visit you will have the opportunity to ask questions and meet members of the clinical team face-to-face. We would welcome the opportunity to meet with you as well.
